How to submit your forms

There are several ways you can submit your forms without visiting the office (due to the Corona/COVID-198 pandemic, do not visit our office until further notice).

The easiest way is to print, complete, sign your forms and fax them to 910.251.7859.
You can also send them to Dr. Birdsong via a special HIPPA-compliant messaging service from your phone
1. Print, complete, and sign your forms
2. Take a picture of them with your phone
3. Call the office (910.763.3034) and ask to have Dr. Birdsong text you a link to a HIPPA-compliant message service
4. You will receive a text message with a link. Click on that link (do not reply to the text). That link will take you to the message service web page. You will be instructed to create a pin # (do not lose this).
5. Once you have created your PIN, you will see a messaging service that looks like a texting program (see image). There is a small paperclip icon on the bottom left (to the left of where you'd type your response). Click it and attach the pictures of your forms (for most, these files will be in your phone's photo gallery). Send the message with your attached files.
You can also print, complete, and sign your forms and mail them. However, your forms must be received in our office before Dr. Birdsong can see you (via Tele-Health). Please mail your forms in well ahead of time (allow 1 week). Our address is: Lori Birdsong MD, 720 N. 3rd St, Suite 101, Wilmington NC 28401.
Lastly, if you don't have access to a printer, our office can mail you the required forms. You can then return them via the options outlined above: by fax (#1), by HIPPA-compliant message (#2), or by mail (#3).
